Overview:

In this April 24 fireside chat, Sean Jeffery, PharmD, BCGP, FASCP, AGSF, Director of Clinical Pharmacy, UCONN, and co-founder of The Coalition for Senior Medication Safety, and Jenn Searls, MHA, Executive Director of Connie (Connecticut's state-designated health information exchange) will explain how the state of Connecticut launched a medication safety tool to address the challenging issue of medication reconciliation across healthcare organizations and settings. In addition to detailing the extensive efforts involved in developing and implementing the tool, Jeffery and Searls will explain how it can serve as an example for other states to undertake similar efforts.

    Sean Jeffery, Pharm.D., BCGP, FASCP, AGSF, is the Director of Clinical Pharmacy at The University of Connecticut, as well as the Co-Founder of The Coalition for Senior Medication Safety.

    Jenn Searls, MHA

    Jennifer Searls, MHA is the Executive Director at Connie, the state of Connecticut’s official health information exchange. She has extensive experience working with health care technology and information systems. Prior to joining Connie, Ms. Searls was the Chief Operating Officer at SOHO Health where she led the effort to scale the former Saint Francis Healthcare Partners to a regional clinically integrated network for the 1,700 providers and five hospitals affiliated with Trinity Health Of New England. Additionally, Ms. Searls built the population health infrastructure as the organization moved to value-based healthcare in its contracts with payer partners. Ms. Searls was previously the Chief Information Officer at ProHealth Physicians. During her tenure, Ms. Searls orchestrated the successful transition of the organization from paper records to an electronic medical record – on time and under budget. In 2011, Ms. Searls co-led a multi-disciplinary team who guided the organization and all of its practices to the achievement of Level 3 recognition as a Patient Centered Medical Home (at the time, the largest single PCMH submission in NCQA history).
